    7. Preceptorship Certificate signed by a certified preceptor of PALES attesting to at least:

      • 10 Assisted Lap Chole cases

      • 10 Performed Lap Chole cases

      • 2 Assisted Lap Appendectomy cases

      • 3 Performed Lap Appendectomy cases

        • Please submit copy of Histopath Report of all PERFORMED cases.

    11. Submit five (5) names of PALES fellows with corresponding email addresses.
      (Names of FELLOWS must be coming from the chapter or from the Fellows practicing in the same area. If there is limited numbers of fellows from the area, submit names of fellows from neighboring provinces. Endorsement given by your preceptors will not be accepted.)

    12. Five (5) unedited video of Lap Chole Performed within the last two years

      • 3 Lap Cholecystectomy

        • ​Follow the PALES standard 4-port technique in achieving the Critical View of Safety (CVS)

      • 2 Lap Appendectomy

    13. Two pcs. (2x2) colored ID pictures.

    14. Application fee - Php1,500.00​

    1. The Committee on Membership and Credentials will make a random check on your institution.

    2. Associate membership is valid up to four (4) years only. All associate members are required to upgrade their membership to fellowship.

    3. Application Fee :             Php1,500.00

Become a PALES Fellow Now

Laparoscopy Preceptor

- are FELLOWS of the Philippine Association of Laparoscopic & Endoscopic Surgeons

  1. Invited by the Board of Directors or Endorsed by their respective chapters

  2. Submitted requirements (see Preceptor’s requirements)

  • REQUIREMENTS

    1. You will be invited and endorsed by your chapter to the PALES board or invited by the PALES board

    2. ​Curriculum Vitae with photograph

    3. Certificate of Fellowship of PALES

    4. Must be a fellow of PCS and PALES in good standing

    5. Must be an active PALES fellow, attending the PALES Annual Convention every year

      • Any preceptor who is INACTIVE or have missed 2 consecutive PALES annual conventions will be automatically delisted from the roster of preceptors

    6. One hundred (100) laparoscopic cholecystectomy cases performed, and twenty (20) laparoscopic appendectomy cases after he/she became a Fellow of PALES with histopathology report.

    7. Five (5) Un-edited video of Laparoscopic Cholecystectomy Performed done in the past 2 years

      • 2 simple, 1 difficult and 1 bail-out cases of laparoscopic cholecystectomy cases and 1 laparoscopic appendectomy case

      • Follow the PALES Standard 4-port technique in achieving the critical view of safety

      • 3-port technique may be used, as long as you can show the steps in achieving the critical view of safety

    8. Submit five (5) names of PALES fellows with corresponding email addresses and hospital affiliation

      • 3 from the area of practice

      • 2 from PALES board /preceptors/trainers of PALES

    1. YOU will be Invited by the PALES Board

    2. Must be a recognized active preceptor of PALES, recommended and trained atleast 5 trainees who became fellows of PALES.

    3. Must have been a faculty in at least 3 basic laparoscopy workshops conducted by PALES

    4. Curriculum vitae with photograph

    5. Certificate of Fellowship to PALES

    6. Must be a fellow of PCS and an active member of PALES in good standing, attending annual convention of PALES every year. 

      • Any Trainer who is INACTIVE or have missed 2 consecutive PALES annual conventions will be automatically delisted from the roster of trainers and preceptors.

    7. Ten (10) un-edited videos of Laparoscopic surgeries performed

      • 3 simple, 2 difficult and 2 bail – out cases of laparoscopic cholecystectomy with (2) simple and (1) complicated laparoscopic appendectomy cases

      • Follow the PALES standard 4-port technique in achieving the Critical View of Safety

      • 3- port technique may be used, as long as, you can show the steps in achieving the critical view of safety.

    2. Letter of intent stating that you want to upgrade your membership from associate member to fellow.
